Vote by Mail
A Vote-by-Mail ballot may be requested for a specific election or, for all elections through the end of the calendar year of the next regularly scheduled general election. The request may be made in person, by mail, by telephone, or by e-mail. Only a voter or a designated member of their immediate family, or a legal guardian, may request a vote-by-mail ballot for the voter.
The deadline to request a vote-by-mail ballot is 5 p.m. on the 12th day before the election. Vote-by-mail ballots must be returned to the Supervisor’s office no later than 7:00 p.m. on Election Day.

Address

Date of Birth

Florida Driver’s License or the Last Four Digits of Their Social Security Number

Voter’s Signature (Written Requests Only)
Vote-by-mail ballots cannot be forwarded. If returned as undeliverable, all subsequent requests will be cancelled.
